Before QuillMD
An hour of notes
after every clinic.
Charting after hours. Templates that never quite fit. The visit, half-remembered at 9 pm.
After QuillMD
The note
writes itself.
QuillMD listens to the visit and drafts the note from what was actually said — every line anchored to the transcript, machine-verified, with a review lane for anything it can’t ground. You sign every note.
- S54M, 3-mo HTN follow-up, home BP ~150/90.

- AHypertension, uncontrolled.
- PAdd amlodipine 5 mg; f/u 4 wks.
- HIPAA-ready

How it works
Four steps.
You’re in the last one.
- 01
Record
Ambient capture in the room. No typing, no dictation — just the visit.
- 02
Draft
A structured SOAP note, written from what was actually said.
- 03
Verify
Every line is anchored to the transcript. Anything it can’t ground goes to the review lane.
- 04
Sign
You review, edit, and sign. Always. The note is yours.
The difference
It only writes
what was said.
Every line is anchored to the transcript. When something can’t be grounded in the visit, it goes to a review lane — not into your note.
The raw transcript
Dr.Good to see you again. How have things been since we adjusted the lisinopril?
Pt.Alright, I think. I've been checking at home like you asked — it's been about 150 over 90 most mornings.
Dr.Let's take a look today. 148 over 92, heart rate 78. And your lungs are clear.
Dr.So the pressure is still higher than I'd like — the hypertension isn't controlled yet.
Dr.I'd like to add amlodipine, 5 milligrams, and recheck in four weeks.

Questions
The ones
doctors ask.
Do I still sign my own notes?
Always. QuillMD writes the first draft; you review, edit, and sign every note. Nothing is filed without you.
How do you keep the note accurate?
Every line is anchored to the visit transcript and machine-verified. Anything it can’t ground from what was actually said goes to a review lane labeled “Suggested by AI — not stated in this visit,” rather than into your note.
Can it write in my note format?
Yes. Upload or paste a sample note and QuillMD learns your structure — headings, style, even tables, checklists, and severity scales. Boxes are only ever ticked when the visit supports them; anything you didn’t address stays blank instead of being guessed.
What about prescriptions?
Rx drafts run through deterministic safety checks — dangerous drug pairs and dose caps included. A critical warning blocks signing until you explicitly review it; the gate fails closed, never open.
Is it HIPAA-compliant?
QuillMD is HIPAA-ready and a BAA is included. Your data is US-hosted.
What happens to the visit audio?
Audio is retained for 90 days after the note is signed, then deleted.
How much does it cost?
$49 per provider, per month — flat, with no per-visit fees.
Which AI model does it use?
A current frontier model. The note is always anchored to the transcript and left for you to sign — the model drafts, it doesn’t decide.
